What To Do If I Lose My Medical Insurance Card?

I’ve lost my medical insurance card and I don’t know my ID number but I have an appointment tommorrow morning. Is there any way to obtain the nessisary information in order to still obtain benefits? Would customer service be able to retrieve it with my social sercurity number?

You might be able to request a new card and print a temporary card or at least the information off the internet if the company has a web-site.
Either way, the doctor’s office should be able to verify your info if you can at least give them the carrier name and your social security number.
