    Normally the date of birth is asked of the patient. Or a second party needs to supply a copy of their POA (power of atty.) to the medical provider. No medical information is to be given out w/o these two questions being confirmed..

    Ask the book keeper at your doctor’s office which insurance companies have the best history of paying their bills. Some insurance companies refuse to pay reasonable claims, or so under-pay doctors that you as a patient will not receive the good medical care you expect to get. Premiums may or may not be higher with the more reputable insurance carriers, but if good care is what you seek, ask your doctor which companies they like.
