You have to be Morbidly Obese. That means over a 40% BMI. You can use a calculator for BMI index in the web, to find out yours. Also you have to suffer from High risk diseases related to obesity, like High Blood Pressure, Diabetes, Heart Problems. It is also good to tell the Dr’s you have tried everything, like Prescription medicine, Atkins diet, Nutrition Classes. Things like that. Those are good requisites to request the Surgery, In some states Medicaid pays for the Surgery. It depends call your Health insurance and find out if is covered… Good Luck is a lengthy procedure, right now I am in the waiting list for this surgery, for the past 4 months, is a long wait…
Ignore the previous tip… If you say you are depress, you may not get it. This is Life Changing and it has to be a decision with the right state of mind. Stay positive when you talk to the Dr’s. tell them you are ready to take control of your life.

every insurance is different. i have kaiser in s calif and it is even different from each hospital, there are requirments and each policy has different ones, you need to be 100 lbs or more overweight if you have a bmi of like 50 you will be approved but you need to prove that you have tried to lose weight before like weo=ight watchres or diet from doc. i was barly over the 100 lb so i need 2 medical needs to get it i have acid reflux and high blood pressure high chosetrol and arthritise you will need a referral from your doc to get the surgery and most require you to go though a class about gastric bypass.
