First of all, Obama is not going to bring in universal healthcare, a fact that many people (including those who voted for him) seem not to realise. He wants to make insurance more available to all. So to answer your question, as there is not going to be a universal healthcare system under Obama in this term of office, the answer is no.
Second, of course universal health-cover sucks. That is why we in Western Europe have it. We think, hmm, our healthcare system sucks. I know, lets keep it. I guess that is the same with Japan and Canada as well.
FACT - the USA spends more on healthcare PER PERSON than any other nation on the planet.
FACT - the US has higher death rates for kids aged under five than western European countries with universal health coverage.
That means that a dead American four year old would have had a better chance of life if they were born in Canada, France, Cuba, Germany, Japan etc, all of which have universal health coverage.

I don't quite understand your question, but in the UK you can claim back any travel expenses you incur when you travel to hospital for your health care.
The UK Government never "tells" anyone to travel accross the globe for medical treatment!
We have private healthcare as well as Universal healthcare in the UK anyway so if you have the money you can get anything you can't get on the NHS through the private system.
If you wanted a new or controversial treatment that is not available in the UK then you have to fund your own travel costs.

I know in Canada, if you need treatment for a medical condition and it isn't offered in your area (say you live in a rural area without a large hospital), your traveling expenses are reimbursed by the government after you have incurred those expenses. I know some things aren't covered, like if you choose to leave the country for medical treatments, and those treatments are offered in the country, they won't reimburse you for that. And I am aware of a person with cancer, who is going to Germany for stem-cell treatment that is not available in Canada and is not being reimbursed because the government doesn't think the science is proven enough to be effective, so they are considering it elective treatment. I don't think your talking about Canada though, I think your talking about the US, and I don't think it makes any sense to speculate about things that haven't even occurred yet, or might not even happen.
