Is the policy through your employer or private? If it’s through the employer, it’s up to them which plan they choose, and whether it’s fully funded or self-funded. Self-funded plans can do a lot of things that funded plans can’t do, and can exclude things too that funded plans don’t. If it’s your private policy you got through BC/BS, you chose that policy.
My company allows a domestic partner, whether it’s heterosexual or homosexual, but you do have to actually live together just like common law marriages. It doesn’t work for people who just share living quarters, like a roommate or your adult child or your parent.
If it’s your private plan, look for a different one. If it’s through your job, talk to them about it.

Here is another way to look at it.
The Mormons with all of their wives. Can you imagine an insurance company insuring say 10 wives, even though they are married and live together, only the 1st wife is allowed on the policy and the others have to get separate medical insurance.
does that make you feel better?

    Are you serious?
Even if there were an insurance policy that allowed a sleaze bag to add a mistress to his policy, I’m sure it would not let you have BOTH a wife AND a mistress. Kind of time to make a choice, wouldn’t you say, buddy?

If you divorce your first wife and marry your mistress, I’m sure you can add her then. I’d first make sure your current wife is agreeable to this arrangement.
